North Star athletics is committed to developing our student-athletes in an educational-based environment. Through a positive coaching atmosphere, student-athletes will be encouraged to expand their athletic abilities.

Our coaches are dedicated to not only teach the fundamentals of a sport, but also develop well-rounded individuals who understand the importance of determination, discipline, sportsmanship and character. 

North Star Middle School Athletics

IMPORTANT! A Sports Physical is required for ALL athletes (Any athlete, e.g., 6th – 8th grader who has never played an extracurricular sport at North Star Middle School).

Returning NS Athletes: A physical interim/update form is required (for returning athletes, e.g., 6th – 8th grader with a sports physical form already on file).

Each North Star Middle School Athlete must complete and submit the required athlete forms:

• Sports Physical Form (First time participant Grades 6-8 OR any new student athlete to North Star)
• Interim Health Update Form  (Returning North Star athlete with a sports physical currently on file)
• Participation Release & Waiver Form
• Code of Conduct Form
• Emergency Contact Form
• Concussion Waiver  /  Review Concussion Info.
• Transportation Waivers:   Passengers Form

• Pay Athlete Fees
