IHSAA Sports Physical Form: A sports physical is required for first-time participants, grades 6-8, or any new student-athlete to North Star Schools. This form can be printed, picked up at the school office and completed at your local Doctor’s Office or Quick Care. Please request a copy of the completed sports physical and submit to athletic director of middle school front office.

Interim Health Update Form: Returning athletes with a sports physical currently on file: Complete this form and submit before the first day of practice.  Print this form or a copy can be picked up at the school office.  Please complete this form and submit to athletic director of middle school front office.

• Driver Form- Print, sign and complete requirements. Return to Head Coach and/or Athletic Director. Driving any athletes other than your own child is not permitted unless this form is completed, approved and on file with AD, in advance to travel.
